DA008 – Information on medically appropriate supplement to authorized scheduled treatment

Purpose of the SED:

This SED is an Information about the medically appropriate supplement to authorized scheduled treatment.

• this is information from the Member State of stay to the competent Member State that it was medically appropriate to supplement the treatment covered by the existing authorization;

• Supplement to the treatment covered by the existing authorisation apply only for the benefits in kind provided in relation with an accident at work or an occupational disease;

Data required:

Specific mandatory fields in DA008 (without which SED cannot be send):

• 5. “medically supplement to authorized scheduled treatment” with a following details on the treatment and authorisation:

5.1 Date of the original authorization of scheduled treatment – which

allows to link the supplement treatment with original authorization;

5.2 “supplementary treatment” – with a description of supplementary

treatment, details on the provider and period of treatment;

• 6.1 “medical report attached” – choice between yes or no.
